Service Description

Welcome to our practice! During your initial 60-minute visit, we will thoroughly discuss your health concerns and goals, laying the foundation for a comprehensive, personalized care plan. We will review lab recommendations and collaborate to select the most relevant testing based on your needs and resources. As a new patient, you will receive a link to complete some required forms, which must be finished before we can confirm your appointment. If you've had any blood work done in the past three months, please bring those results with you, or ensure you have access to your patient portal to retrieve them. We look forward to seeing you soon!
